I am not sure what options I have missed, so please look into this.
The page title, breadcrumb, grid, list icons are missing for shop page, product categories pages, I check the code they are actually there but hidden for some reason so I am assuming I have to do something in theme options but cannot figure it out.
About the page title and breadcrumb, you can not see them because color is white. You can change the breadcrumb colors in Theme Options > Color Scheme tab > Breadcrumb Colors section. But it will affect to all breadcrumb layouts. You can also change the breadcrumb layout of the shop/product category page by editing your shop page. In Page Options, you change Breadcrumb Layout to Default or a layout which you want.
About the Grid List icons, you go to WooCommerce > Settings > Products > Display. You check to make sure that you don't disable the Catalog view option. If it still does not appear, please try disabling your third party plugins one by one, clear cache and check it again.
